The NCLEX-RN (National Council Licensure Examination for Registered Nurses) is the licensure exam every registered nurse in the United States must pass, developed and administered by the National Council of State Boards of Nursing (NCSBN). It is a computer adaptive test (CAT): the difficulty of each question adjusts based on your previous answers, and the exam ends once the computer can determine with statistical confidence whether you are above or below the passing standard. Candidates answer a minimum of 85 and a maximum of 150 items within a 5-hour time limit.
In addition to these categories, the exam includes 18 clinical judgment case study items (three sets of six) plus standalone items measuring the Clinical Judgment Measurement Model, NCSBN's framework for how candidates recognize cues, analyze information, prioritize hypotheses, generate solutions, take action, and evaluate outcomes.
The NCLEX-RN tests applied clinical judgment, not just recall. Many questions present a patient scenario and ask you to prioritize, recognize warning signs, or decide on the next best action. Practising realistic scenario-based questions builds the clinical reasoning skills the exam is actually designed to measure.

About the real test
The NCLEX-RN is a computer adaptive test with a minimum of 85 and maximum of 150 items across 8 NCSBN Client Needs categories. It tests applied clinical judgment through patient scenarios, not just recall.
Pharmacological and Parenteral Therapies carries the most weight
At 13-19% of items, this is one of the largest categories. Prioritize the "Rights" of medication administration, high-alert medications, and recognizing adverse reactions.
Think prioritization, not just facts
Many questions ask what to do first. Use frameworks like ABC (Airway, Breathing, Circulation) and Maslow’s hierarchy to work through prioritization questions systematically.
Safety and Infection Control questions have a clear right answer
Standard vs. transmission-based precautions, hand hygiene indications, and restraint use all follow well-defined rules. These are some of the most learnable points on the exam.
